Ever felt you weren’t quite sure what the LVEF was on echo? So, you ordered a formal echo and had to wait six hours? Fret no more, after this section on qualitative (or global) assessment of LV systolic function you’re guaranteed to feel more comfortable about your ability to assess the LV systolic function.

Key Learning Points:
–Actual value of LVEF isn’t usually helpful — categorize LV function:
LV Ejection Fraction (LVEF):
- >70% Hyperdynamic  (squeezes almost completely empty)
- 55-70%Â Normal (decrease in LV chamber 40-50%)
- 45-54%Â Mild dysfunction (good, but not quite right)
- 30-44%Â Moderate dysfunction (some squeeze)
- <30%Â Severe dysfunction (barely moving)
In shock/hypotension, think about LVEF by category:
- Hyperdynamic —Â NOT the cause of shock
- Normal/mild — UNLIKELY the cause of shock, but LV might not be compensating appropriately either
- Moderate/severe — Likely CONTRIBUTING to shock, if not causing malperfusion
Qualitative assessment can be done by a non-cardiogist accurately.
Don’t be misled by QUANTITATIVE measure
